Fuze Health offers a strong set of employer-paid insurance benefits to help protect you and your family when the unexpected happens:

  • Basic Life Insurance provides a lump-sum payment to your beneficiaries if you pass away, helping support your loved ones financially during a difficult time.
  • Accidental Death & Dismemberment (AD&D) adds extra protection if you die or are seriously injured in an accident, such as losing a limb or eyesight.
  • Short-Term Disability Insurance helps replace part of your income if you’re unable to work for a short time due to a non-work-related illness or injury.
  • Long-Term Disability Insurance continues income replacement if a serious condition keeps you out of work for an extended period, after short-term benefits end.
  • In addition, you may choose to purchase Voluntary Life and AD&D Insurance to add extra coverage based on your personal needs.

Together, these benefits provide important financial protection and peace of mind for you and your family.

Basic Life and AD&D

Life & AD&D Insurance provides financial protection for you and your loved ones in the event of death or a serious accident. Life insurance pays a lump-sum benefit to your chosen beneficiary, while AD&D offers additional coverage for certain accidental deaths or serious injuries. Together, this coverage helps ease financial stress and provides peace of mind when it matters most.

You simply need to name the beneficiary of your choosing and change it as needed.

Voluntary Life and AD&D

We offer you the opportunity to purchase additional Life/ Accidental Death & Dismemberment coverage for yourself, your spouse/domestic partner, and child(ren). You must enroll yourself to elect benefits for your dependents. Employees are responsible for all of the premium costs of voluntary life insurance. Premiums will be deducted automatically from your paycheck. See Voluntary Life and AD&D Summary for rates and additional plan information.

For late enrollees or coverage in amounts above the guaranteed issue limit, you will need to complete an Evidence of Insurability (EOI) form with additional health information to the carrier for review. EOI allows the insurance carrier to determine the risk in offering you additional coverage. In some cases, a physical exam or blood work may be required.

To complement your MetLife life insurance coverage, for those employees enrolled in the Voluntary AD&D benefit, you have access to Travel Assistance services, a useful program giving you and covered family members access to travel assistance professionals at AXA Assistance USA, Inc. when traveling within the U.S. or abroad. They have the expertise to help when you need emergency medical, travel and personal assistance. Please refer to the Travel Assistance flyer to the right for information.
