Employee Benefits Overview / Eligibility Requirements
At Fuze Health, we are committed to offering exceptional benefits that support the personal and financial well-being of you and your family.
This marks the first time that employees will enroll under a unified Fuze Health benefits program.
Your benefits will begin on January 1, 2026. The benefit elections you make during this Open Enrollment period will remain in effect for the entire 2026 calendar year, unless you experience a Qualifying Life Event (such as marriage, birth or adoption of a child, or loss of other coverage).
If you experience a Qualifying Life Event for 2026, you will have 30 days from the date of the event to make changes to your benefits. To do so:
- Log in to Okta (Alto), Entra ID (Truepill) or JumpCloud (LetsGetChecked) and select the Workday icon.
- From Workday’s Home Page, open the dropdown menu in the upper-left corner and select Benefits.
- Click the PlanSource Portal link to update your elections.
If changes are not submitted within 30 days, you will need to wait until the next Open Enrollment period to make updates.

Eligible Requirement and Dependents
If you work 30 hours or more per week, you are eligible to enroll in the benefits plans.
Eligible Dependents
You may cover your dependents under many of the benefit plans as long as they are one of the following:
- Your spouse or registered domestic partner
- Your child(ren) up to age 26 regardless of student or marital status
- Your handicapped child(ren) regardless of age if incapable of self-sustaining employment, and if the handicap began before the limiting age
Domestic Partner Definition
A domestic partner is a person who meets all of the following criteria:
- Both partners are eighteen (18) years of age or older and mentally competent.
- The partners are not related by blood in a way that would prohibit marriage under applicable law.
- The partners have a close and committed personal relationship and are each other’s sole domestic partner, not married to or in a domestic partnership with anyone else.
- The partners have shared the same regular and permanent residence in a committed relationship for at least six (6) months and intend to continue doing so indefinitely.
Employees must notify their employer in writing within thirty (30) days if they are no longer in a domestic partnership. Domestic partner benefits will end when the partnership terminates.
Please note:
- All domestic partnership benefits are subject to imputed income in accordance with IRS regulations, and employees may be taxed on the value of coverage provided to their domestic partner.
- If you and your partner can register as domestic partners in your state, such registration will also qualify under this definition.

How Do I Enroll in Benefits?
New hires will receive a welcome email following orientation with a link to the PlanSource enrollment system. Within PlanSource, you can enroll yourself and eligible dependents in the plans that fit you and your family best. For more information about the plans offered, please go back to the homepage or click on the corresponding tab at the top of this page to review plan designs, required notices, evidence of coverage, documents and much more.
